Positive Life: Having a conversation about Advance Care Planning

Advance Care Planning discussions can be difficult, they encourage us to think about what is important in our lives and to how consider illness and medical treatment can affect our quality of life.
An advance care plan can also assist us in feeling a greater sense of control and are an invaluable resource for our families to help them understand treatment wishes and guide them to make treatment decisions if you are not able to.
In the session we will explore what an advance care plan is and why we should all having one and how to start the conversation.
This event will be held on the Microsoft Teams platform. How you register and join the event might look a little different to what you're used to – but it should be quicker and easier!
